Highlight Video Request Form

This form must be completed and submitted at least four (4) weeks prior to the requested due date. The Creative Admin will schedule a time for someone to come film on a Wednesday.

Please note:
- Videos are 15-30 seconds long.
- Videos can be aired during weekend services (countdown timer ONLY) or through our social
media accounts. If the video is needed for social media, please complete a Social Media Request
Form as well.
- Videos are a recording of a person giving an announcement of an upcoming class or event.
- Videos will have airdates 2 weeks prior to class or event.
